Our primary purpose is to document the patient's course of care and to provide a medium of communication among health care professionals for current and future care.  In order to fulfill these purposes, significant amounts of data must be revealed and recorded.  The patient must be assured that the information shared among the health care professionals will remain confidential.

The health record is the property of the health facility and shall be maintained to serve the patient, the health care providers and the institution in accordance with the legal, accrediting and regulatory agency requirements.  The information contained in the health record belongs to the patient, and the patient is entitled to the protected right of information.  All information contained in the health record is confidential, and the release of information will be closely controlled with the establishment of written policies for the collection, storage and dissemination of information and ongoing educational programs for all staff and personnel to enforce these policies.
